Alcohol Consumption and the Long-Term Incidence of Cataract and Cataract Surgery: The Blue Mountains Eye Study

Purpose

To assess whether alcohol consumption is associated with the long-term incidence of cataract or cataract surgery.

Design

Population-based prospective cohort study.

Methods

A total of 3654 persons aged 49+ years were examined at baseline and 2564 were re-examined after 5 and/or 10 years. Lens photographs were taken at each visit and assessed using the Wisconsin Cataract Grading System by masked graders. An interviewer-administered questionnaire was used to collect information on alcohol consumption.

Results

No significant associations were observed between alcohol consumption and long-term risk of nuclear, cortical, and posterior subcapsular cataract. However, after adjusting for age, gender, smoking, diabetes, myopia, socioeconomic status, and steroid use, total alcohol consumption of over 2 standard drinks per day was associated with a significantly increased likelihood of cataract surgery, when compared to total daily alcohol consumption of 1 to 2 standard drinks (adjusted odds ratio [OR] 2.10, 95% confidence interval [CI] 1.16-3.81). Abstinence from alcohol was also associated with increased likelihood of cataract surgery when compared to a total alcohol consumption of 1 to 2 standard drinks per day (adjusted OR 2.36, 95% CI 1.25–4.46).

Conclusion

A U-shaped association of alcohol consumption with the long-term risk of cataract surgery was found in this older cohort: moderate consumption was associated with 50% lower cataract surgery incidence, compared either to abstinence or heavy alcohol consumption.

Tender Loving Care

We just had a wonderful gentleman who drove a 100 miles from beyond palmdale to have Laser vision correction with our Beverly Hills LASIK doctor. After the ride he was tired and need a little pampering with an arnold palmer.(yes we serve that to our patients)
From being legally blind at -6 Diopters he went to seeing 20/40 in a matter of 10 minutes.
He still needed a little TLC or tender loving care. So our wonderful caring staff, brought out a pillow and warm blankets and converted one of the consult rooms into a bedroom for him so he could take a nap. That helped him a lot. TLC improved the vision too.
Now he was 20/20

Intralase assisted Intacs

Newer technology like intralase has more benefits than just fashioning LASIK flaps. At our Beverly Hills surgical center we are happy to use intralase laser from abbott medical optics. We can use this Intralase to make channel in the keratoconus cornea. In this channel Intacs ring segment can be inserted. This channel is more snug than the one made by mechanical devices. Intacs are used to lend support to the weak cornea and help restore the prolate shape. This can decrease the higher order aberrations and help the person see better.

Intralase is a femtosecond laser which can be used to fashion a lasik flap. It is computer controlled, though a competent surgeon is still essential. The flaps can be visualized while being fashioned on a computer monitor.The newer faster Intralase gives better flaps with less side effects. Q. I slip in a reading lens most days in one eye but prefer it out if doing reading for a day. How do you avoid leaving me with that slight negative one gets of a reading lens in one eye?
A.The eye is like a camera. To focus at different distances one needs to change the focal length of the system. In childhood and youth (those wonderful years) the lens is able to achieve this by changing its shape. The lens is made of the similar material as our hair.From the time we are in the womb, lens material gets added every year. Therefore the lens becomes thicker and more rigid and unable to change its shape and hence the focal length. When our hair grows longer we cut it off. Unfortunately no one gave the lens a shave: that is till now when your's truly will play the role of a barber and shave of the lens using pressure waves;) The non working lens will be replaced by either an accommodating lens or a multifocal lens. These lenses have more than one focal distance allowing one to see far and near.They are made of bio compatible material like acrylic or silicone.

Currently you are using a contact lens to change the focal length. Of course that has its own risks and hassle. Instead an intraocular lens will achieve this. This is NOT like a multifocal contact lens.

Lasik just changes the shape of the cornea and still has only one focal length, whereas PRELEX has more than one focal length

Yeah one can autograph the crutch and make it glamorous.
When blind can see again is the most exciting moment.
I restored vision in a blind eye. She and her mother were crying with joy. Her sister in Oregon, who is a family physician was amazed. After 50 odd years of not seeing out of that eye, light entered and traveled to the brain. The doc said the books have written its not possible.
That's what we are doing- changing the books one page at a time.

New Technology Allows You to See Near and Far Without Glasses

If you have undergone cataract surgery, there are three primary methods that can allow you to see near and far. These include:

Monovision
Multifocal Intraocular lens (IOL)
Accomodating IOL

Monovision
This is a process of setting the focus of one eye on close-up objects and the focus for your other eye on far-away objects. In doing this, your brain combines the two images to create a clear picture.

Multifocal IOLs
These include the ReZoom, ReSTOR, and Tecnis lenses. Multifocal IOLs are lenses that have concentric rings of varying focal power, like a bullseye. Some rings provide focus for near objects, others for mid-range objects, and some for seeing objects at a distance.

The Tecnis Multifocal Foldable IOL also has concentric rings for different distances. It received FDA approval in January, 2009 for use in cataract surgery, where the natural lens is replaced. In the clinical trials most participants achieved 20/25 distance vision or better and 20/32 or better for near vision. It is designed using Wavefront data - the same precise level of information that is gathered when you are having Wavefront LASIK.

Accommodating IOLs

These lenses are designed to change focus. An accommodating IOL allows you to see distant objects, and then is can change focus and allow you to see objects that are close up. The only accommodating IOL approved in the United States is the Crystalens.

Signs and Symptoms of Presbyopia

As you grow older, you may find that is more difficult to read, use the computer and focus on near objects. The reason is presbyopia. This is a medical condition where people begin to lose the ability for their eyes to focus.
The earliest warning signs can be seen between the ages of 40 and 50, though some experience symptoms years before.

Difficulty reading and blurred near vision are two of the most common signs of presbyopia. Some individuals may also experience headaches and eye fatigue as well.

The most common symptoms include:

Hyperopia, or farsightedness. Objects at a distance are clear, but objects close up are blurry
Far-sightedness increasing with age
Difficulty focusing on nearby objects
Difficulty reading
Experience eyestrain or headaches from prolonged reading or close work
Holding books or magazines at a distance to read clearly

One often faces many problems being a contact lens user. Dry eyes, irritation, and blurred vision are just a few of the inconveniences that wearing contact lenses can bring. Not to mention the hassle of having to worry about always carrying an extra pair on you and having to remove them periodically and clean them. Wearing contact lenses eliminates the need for uncomfortable and bulky glasses, but it brings a lot of difficulties as well.

For someone who wears contact lenses, they are constantly reminded that they have a foreign object on their eyes- ouch :(. For a college student or for someone who works in an office setting, staring at a computer screen all day can cause dryness and tiredness. The dryness causes one's eyes to become red and watery, and can lead to headaches. And the tiredness causes one to have to frequently take a break. Eye drops help in these situations, but they only provide temporary relief. This affects one's productivity because they are not able to function as well or as effectively as they should be able to.

Another problem which contact lenses bring about is blurred vision. This happens sometimes when the contact lens gets dirty or when it becomes too dry. This can cause much inconvenience and can be dangerous in situations such as driving a vehicle. Imagine having to drive at night with blurry contacts, this can be a great risk and can put your own life and other's lives in danger. Sometimes taking the contacts out and cleaning them does not even help. One must throw the pair out and get a brand new pair, which can become expensive if the problem keeps reoccurring.

For a contact lens wearer, another worry is always having to have an extra pair of contacts or glasses on them as well as cleaning solution and eye drops. For someone who relies on contacts to be able to see, they cannot take the risk of not being prepared if one of their contacts falls out, or if it becomes too blurry to see, or if their eyes get too dry. Therefore, they always have to carry these items with them. This is an inconvenience for everyday life, not to mention when travelling.

Have you ever played football with contact lenses? Having my contact lenses fly away on a tackle and than not knowing where to run was embarrassing and frustrating.

Contact lenses are supposed to be a more convenient alternative to wearing glasses, but they bring about many problems from dryness to irritation to blurred vision and much more. With all of these problems, wearing contact lenses can be very uncomfortable. Always having to worry about your contact lenses is not a convenient way to live one's life. For someone who wears contacts, they are always reminded of it and it is a fact they cannot escape. For these reasons, I hate contact lenses! Wish there was a solution... Hmmmm...

Dry eyes are less common

Surgeons who use epiLASIK have reported fewer instances of dry eye syndrome as a complication. One reason may be because the thinner corneal flap does not impede the normal processes of the corneal nerves that trigger the responses necessary to moisten your eye's surface.

EpiLASIK differs from LASIK because of the type and thickness of the corneal flap. The flap cut is so thin that it does not penetrate the actual cornea substance, only its outer layer, known as the epithelium. An epithelial separator is used to separate the epithelium sheet from the eye. Because epiLASIK does not actually penetrate into the superficial layers of the cornea like LASIK does, it is less likely that you will develop or worsen dry eyes with this procedure.

Differences in Types of Intraocular Lenses (IOLs)

Intraocular lens (IOL) implants are used to correct poor vision, whether it is clouded by cataracts or blurred by presbyopia.

With this type of surgery, your eye's natural lens is replaced by an intraocular lens implant. A monofocal intraocular lens may be implanted to give you a clear point of focus, at a distance or close up.

Types of Multifocal IOLs

Because of their ability to correct vision at all distances, multifocal IOLs are the most preferred IOLs. There are three main types:

Multifocal refractive

Multifocal refractive IOLs, like the ReZoom, are crafted with several optical zones on the intraocular lens. These different zones allow for various focal points, which improves your vision:

At a far distance
At intermediate distances
At near distances

Apodized diffractive

Apodized diffractive IOLs, like the ReSTOR have gradual diffractive steps on the implant that creates a smooth transition between focal points. This IOL also bends incoming light to the multiple focal points, which enhances vision in different light conditions. Acrysof has recently improved the optical properties of its ReSTOR lenses and released a new version, known as ReSTOR +3.0D. The Khanna Institute is proud to offer this new lens which achieved patient satisfaction rates of 95 % in clinical studies and gave a fourfold increase in the number of patients achieving 20/20 vision or better.

Accommodative

A newer type of IOL, the accommodative, includes lenses such as the Crystalens. These IOLs have one focal point, but act like your natural lens, changing shape to move the single focal point to bring objects at varying distances into focus. Recently, Bausch and Lomb introduced Crystalens HD, which improves depth of focus to improve near vision while maintaining high quality intermediate and distance vision. New IOL Technology

The Khanna Institute of LASIK and Refractive Surgery is proud to announce it is offering the newest version of Acrysof's ReSTOR intraocular lens (IOL), the Acrysof ReSTOR +3.0 D. The ReSTOR +3.0 D received approval by the Food and Drug Administration on December 23, 2008 for the correction of presbyopia in cataract patients. In clinical trials, the ReSTOR +3.0 D achieved a superb patient satisfaction rate, with 95 % of recipients saying they would have get the lens again. Nearly four times as many patients achieved 20/20 vision with ReSTOR +3.0 D as with previous versions of the ReSTOR lens.

When the natural lens in one or more eyes becomes cloudy with cataracts, the only available treatment is replacing the lens with an artificial lens. Early replacement lenses only allowed recipients to see clearly either up close or at a distance, but ReSTOR and other advanced IOLs can give patients a full range of vision following surgery. The ReSTOR lens can also be used to correct presbyopia, a condition that affects many Americans aged 40 and older, typically requiring the use of reading glasses or bifocals. Because of its superior optical qualities, the ReSTOR lens offers the chance for people to continue performing all their day-to-day tasks without the use of reading glasses or bifocals.

The first troubling sign of vision loss for Catrina White occurred while she was riding a tandem bicycle with a deaf and blind person behind her. White hit a pothole she didn't see.

be about right. One good alternative way to get this single red line correctly aligned is to only look at the top
view (where the red cross is) and get the iris of the patient into the best possible focus. This method will allow
you to concentrate on one picture only whilst aligning the device. This makes it quicker and easier as well.
Other factors of importance are tear film and shadows of the nose and eye lashes.
All these success factors lead to the following best practice for taking measurements:
1. align the instrument to about the right position: get the red cross on the four spots and get the iris into
sharp focus.
2. now ask the patient to BLINK and OPEN EXTRA WIDE
3. quickly make the final adjustments of the device to get the red cross right and the iris sharp
4. and push the button on top of the joystick
The data will now be processed and as soon as that is done the Verify And Edit Screen will appear and with
that a pop up with one overall quality percentage and 5 specific ones.
The Overall Quality is an important indicator and should be
greater than 90% in general. Also important is Motion
Compensation Quality which should also be more than 90%.
The Motion Compensation Quality indicates how well the
patented eye tracker system had been able to follow the eye
movement during the procedure. Eye tracking is critical for
combining all the data points of the different images that are
taken in one measurement.

The cornea is the front surface of the eye. Similar to windshield wipers eyelids spread tears across the cornea to yield a polished optical surface and hence better vision.

The Waterworld of the Eye:
Like a tap pumping out water, the tears are secreted by the lacrimal glands and accessory lacrimal glands. Luckily there is no bill to pay. The eye lids act as windshield wipers which spread the tears evenly across the cornea polishing the optical surface. Finally the tears go down the drain thru puncti (small openings in the lid margin) to the nasolacrimal duct and into the nose. That is why when you put drops in the eyes sometimes you get a funny taste in the mouth.

Importance of Tears:

The tears are a combination of aqueous, lipid and Meibomian gland secretions each of which must be in proper ratio and volume. The natural tears are also an important source of minerals enzymes and antimicrobial substances like the lysozymes. These various substances are important to protect the eye from infection by killing microbes on the surface of the eye. Natural tears also provide nourishment to the top layers of the cornea. When the body is not able to make enough tears to protect the surface of the eye, cells on the front part of the cornea become sick or even die leading to a variety of symptoms that affect both comfort and vision. Prolonged reading or computer work can often cause evaporation of the tear film. Dry eyes can also be caused by hormone changes, allergies, auto-immune disease, eye surgery, contact lens wear, eye infections and drugs such as oral antihistamines, anti-depressants or hormone replacement therapy. Environmental factors such as exposure to cigarette smoke or other chemicals can also lead to problems.

ROCHESTER -- Dr. Robert W. Hollenhorst, 94, a Mayo Clinic physician and pioneer in neuro-ophthalmology, died Thursday (Jan. 10, 2008) in Rochester.
Born Aug. 12, 1913, in St. Cloud, Minn., he attended St. Cloud State Teachers College, St. John's University, and the University of Minnesota Medical School. During World War II, he served in the Army Medical Corps in the Pacific, and was awarded a Bronze Star for his medical role in several amphibious landings in New Guinea and the Philippines. Following his discharge in 1946, Dr. Hollenhorst began his career as an ophthalmologist at Mayo Clinic and professor at the Mayo Graduate School of Medicine. He retired in 1979 and lived in the Brainerd Lakes area until moving back to Rochester in 1996.
He was a member of many medical boards and associations, and served terms as president of the American Ophthalmology Society and vice chairman of the American Board of Ophthalmology. In 1986 he was awarded the prestigious Howe Medal by the AOS.
Dr. Hollenhorst was founder of the Minnesota Pre-School Survey of Vision and Hearing, and for 30 years was consulting ophthalmologist to Minnesota State Services for the Blind. For his contributions to the state, Gov. Arne Carlson declared April 24, 1991, "Dr. Robert Hollenhorst Day."
Some of his research described a phenomenon now known as "Hollenhorst Plaques." These cholesterol bodies, sometimes found in the blood vessels of the retina, are useful in diagnosing the risk of stroke and other circulatory problems.

Santa Clarita Valley is a very nice calm area to live in. Nothing major has occurred here before until Sunday, October 20^th. Many different cities were burning due to fires caused by the strong winds. There were 4 fires just in the city of Santa Clarita. These cities are Canyon Country, Valencia, Stevenson Ranch, and Sand Canyon. All areas had many residents evacuate their homes. This was a very scary sight. Any direction one was to look in all you could see was dark heavy smoke. It was nearly impossible to breath. From certain areas every direction you were to look, one would be able to see the actual flames from the fires. It was even possible to see homes that were on fire. Each fire burnt thousands of acres. All the thick smoke, ashes, and wind made it nearly impossible to see.

Wearing contacts at this time was very uncomfortable. My contacts became very dry and irritating. I didn’t want to take them off because I did not know where my glasses were, and I didn’t have much time to look for them. I then realized I should get lasik surgery done. Although one can never be prepared for an emergency or an event like this one, it would be one less thing to worry about. I would request Dr. Khanna to give a rebate to all people affected by the fires in Los angeles and surrounding areas.

Wearing contacts can be such a headache, literally. I need to have LASIK Surgery in order to stop worrying about putting in my contacts daily. I recently went on a trip to San Francisco and of course we all wanted to go sightseeing. Unfortunately, one of the days I was so tired that I forgot to put in my contacts! With my luck we actually had a perfect view of the Golden State Bridge on the day I couldn't see. Everyone was talking about it, but I was just trying to focus on seeing the bridge. All day I had to stress my eyes, it was such a pain. Eventually, I got a really bad headache from not wearing my contacts. It was horrible, because as much as I wanted to enjoy my time and try to see everything I couldn't handle my headache.

Earthquake ready

I hate earthquakes!! I live in Los Angeles County, California. Not too long ago we had a 4.5 earthquake, and it completely left me in shock. I unfortunately wear contacts and glasses. Without them I cannot see very clearly at all. At the time of the earthquake the last thing on my mind was to go put on the contacts, because I could not find my glasses. I was way too scared to stay in my room and look for them, so I just ran out with out them. It was very hard to see, but I knew that I just had to deal with it. Even if you are not living in California, emergencies and earthquakes can happen at anytime, whether we are ready or not. Recently my friend got caught in a tornado in Chicago (the windy city) I want to be able to at least see clearly when things like this happen. The only way I can make that happen is by getting LASIK Surgery.

Love the Dolphins

The beach is one of my favorite places in the world. I recently visited Malibu Beach, and had an amazing time. It was the perfect weather and all of my friends were there with me. We had so much fun. At one point one of my friends yelled out “look dolphins!” I love dolphins. Everyone was talking about how pretty and close to shore they were. Unfortunately I could not see them, because I forgot my contacts! I realized that I didn’t put on my contacts when I got in the car, but I figured I would not need them. I thought it was a good thing that I didn’t put them on, because now they would not bother my eyes when I got in the water. So in a way it was good that I didn’t wear them, but at the time I didn’t get to see the dolphins everyone was talking about. If I had LASIK surgery I would be able to swim in the water without my eyes being bothered, and at the same time I would have seen the dolphins.

College Life

I am a student at College of the Canyons in Valencia, Santa Clarita. I take all morning classes, which means I have to wake up early and partying in Santa Monica is restricted. I wear contacts in order to see clearly and at a distance, but in the mornings I am in such a hurry that sometimes I forget to put them in, and don’t remember to bring my glasses. When I get to school and realize that I can’t see the board and don’t have my glasses, I just leave class at times. I figure that it is pointless for me to be there, because I can’t see the lecture the teacher is writing on the board. I hate stressing my eyes just to see what is being written. This really makes me want to get surgery on my eyes. I think LASIK surgery would really make my life a lot easier.

Tired of Contacts?

Wearing contacts is such a hassle!! Having to put them on every morning and taking them off at night can become one of the most irritating things to do. I am a very busy person, and sometimes I can easily forget to put in my contacts. There have been times when I have to drive back home to put on my contacts. There have also been times when I just do not want to put them in, because it is so early in the morning the contacts burn my eyes. I have actually needed to take the contacts out due to the burning in my eyes. Taking out the contacts is also really annoying. When I get home late at the night, the last thing I want to do is take them out. If I sleep with them in my vision the next day is really blurry. At times I just want to watch t.v. at night, yet my contacts are out so I can’t see. Wearing contacts really bothers me in many different ways, so now I am looking forward to have LASIK Surgery, at the Khanna Institute of LASIK. I get to choose to have the surgery at either the Valencia or Westlake Village office. Plus whats great if I get transferred to Fullerton, they even have offices in Costa Mesa and Corona. Khanna LASIK, here I come.

The event was produced by an organization that is real popular with
the entertainment industry (An environmental group called "Plug in
America") The lady in charge - had her vehicle customized
(a Toyota Prius that gets over a 100 mpg and her house is "grreen &
has solar energy & and even provides extra electricity back to the city!!!
On the Malibu pier a movie was screened. The movie had Mel Gibson (who bought my
friend's house in Serra Retreat) He talked about the importance of
being "green" etc.

just got back from the Malibu Pier event. (On the way back, on Agoura
Rd. a car hit a deer. The car kept going, so I had to pull over, and
the poor deer was still alive. I called the SPPA and they finally got
there, but by theat time the deer staggered off into the hills.

WASHINGTON, Aug. 2 (UPI) -- U.S. research is expanding the theory that vision is processed in the brain along circuits made up of neurons.

Scientists at Georgetown University Medical Center have discovered visual information is also processed in a manner similar to propagating waves oscillating among brain areas.

"What we found is that signals pass through brain areas like progressive waves, back and forth, a little bit like what fans do at baseball games," said Associate Professor Jian-young Wu, the study's corresponding author.

Just as a "stadium wave" travels through the crowd, a collective pattern emerges from the activities of millions of neurons in the visual areas, he said, adding, "It simply makes sense that brain function is the result of large numbers of neurons working together."

The finding challenges longstanding theories about how the brain processes sensory information, Wu said.

"One traditional model theorizes that neurons are hooked together into specific circuits," he noted. "However, new imaging methods tell us that there are more than just circuits."

The research that included Weifeng Xu, Xiaoying Huang and Kentaroh Takagaki was reported in the July 5 issue of the journal Neuron.

This can explain why traditional methods of vision measurement are not enough. And why people with similiar vision still see differently.

Being an Athlete is not an easy job, when having to worry about wearing glasses or contacts. It is almost impossible to play at one’s best ability, when their vision is not perfect. I personally play soccer, and have to wear contacts. Although, wearing contacts increases my vision it also has an effect on it. Soccer is a very physical sport and it includes a lot of running. During my games sometimes my vision gets blurry or my eyes become very dry.The Santa Ana winds blowing in Los Angeles are no help. It becomes very hard for me to focus on the sport, when my eyes are bothered by my contacts. How will I be able to bend it like Beckham? There are times when my contacts move out of place, and I just can’t continue playing the game until I fix the contacts. I would love to play soccer without having to worry about my eyes being bothered - and become the next Mia Hamm:) For this reason, I am highly considering getting lasik surgery done!

Washington, DC- July 12, 2007- The US Food and Drug Administration today approved monovision Lasik. Monovision is the process of correcting one eye for distance vision and one eye for near vision to reduce the need for reading glasses or bifocals for patients affected by presbyopia - the inability of the eye to focus on near objects. For most people the brain adjusts to the difference in perception between the two eyes. Monovision can be provided with contact lenses or laser eye surgery.

Monovision is already available as an off-label use of the Lasik laser. An off-label use is when a medical device already approved for one procedure is used for another. Off-label use is a common method by which doctors put medical devices to the best use for patients. Please Describe Your Situation::
I am having the surgery done on the 21st of this month but still so scared and not sure of which way to go। I am scheduled for intralase and was told to call if I chose the microkeratome instead। I have been up and down with this choice and need some advise. The cost between the two is a 2000 dollar difference. I don't want to sacrifice my eyes for the better deal, but think that is a bit much for the use of a different machine. Will I still get a good quality surgery if I chose the micro instead. Is is better to have the laser do it all or can you even tell me the difference? My eyes are only a - 1.50 and -1.25.....do you know the chance or percentage that I may face an enhancement. That has a lot to do with which option to take as well I think. Any information will help me. Thank you so much.

These are issues you need to discuss with your surgeon. 2000 dollar difference may not be worth the trouble. Trust in the surgeon is of paramount need.
